How to contact Adult Services online, by email, phone or post, how to make a referral.

The Adult Services Referral Team (ASeRT) is here to help you refer yourself or an adult you know for any aspect of social care. This includes care at home, occupational therapy, rehab and enablement, sensory impairment services, and telecare.

We know that asking for care or referring someone you think would benefit from care isn't always easy, especially if you're unsure of the types of care available or necessary.

Our team are experienced as the first point of contact for care referrals and will make sure your unique situation is handled confidentially. We will work with you to get as much helpful information as we can (such as medical conditions, GP contact details, and personal circumstances), answering any questions you have.

You can make a care referral online, by email, over the phone, or by post. GPs can refer patients using SCI gateway. Anyone can make a referral including family members, hospital nurses, social workers, neighbours, and a person can also refer themselves.

We will then pass your referral on to the appropriate care service to ensure that the right care is given at the right time.

We will respond to care referrals within 1 working day and make sure it is passed to the relevant care service swiftly.

For referrals regarding community meals or community alarms, you can contact those services directly online and they will respond to you within 2 working days.

Emergency contact information

Emergency social work services provide assessment and intervention in emergency situations to relieve acute risk and are too urgent to wait for social work offices to re-open.
