Local area co-ordination is a way of helping people with learning disabilities to live good lives in their community. We will work with organisations relating to social work, housing, health, education, employment and transport, so we can offer you independent advice and support.
Once you are linked to a local area co-ordinator, you and your family have access to information and support and you can build up informal and formal relationships that will connect you with the services in your area.
Local Area Co-ordination - what we do:
- We will listen to you and your family about your wishes for the future;
- We believe that you and your family are in the best position to identify your own needs and we will help you find the best solution to any problems or concerns you may have;
- We will keep in regular contact with you and your family; and
- We will support you and help you get access to the services you need and choose.
