Joint Future Annual Report 2004-05

The first Joint Future annual report has been produced by Renfrewshire Joint Care, a consultative partnership body comprising Renfrewshire Council and NHS Greater Glasgow and Clyde. The report comments on the partnership's effectiveness for developing the Joint Future agenda which is concerned with joint working between social work and health to improve services and to improve the lives of those who have community care needs.

As well as providing brief comments on the partnership's performance in 2003-04, a more detailed set of information covering Joint Performance Information and Assessment requirements for 2004-05 is also set out. In addition the annual report gives an overview of the key strategic and service issues facing the partnership over the next 12 months. In replacing the Joint Community Care Plan the report now comments on the service developments and proposed action for each of the main community care groups.
